Expert Strategies for Managing Lymphedema in Cancer Care

Breast cancer-related lymphoedema is a serious and concerning condition that can occur after breast cancer surgery, lymph node excision, or radiation therapy. The underlying pathophysiology includes lymph drainage failure and hemodynamic imbalance. This MOH Talk “Navigating the challenges of lymphoedema in cancer care: expert insights” brings together medical experts from all over the world to discuss state of the art topics in the field of lymphatic disorders. On October 28, 2023, Drs Linda T Miller and Vincent Singh Paramanandam discussed treatment and prevention of lymphoedema in breast cancer patients.

This MOH Talk was co-moderated by Dr Mohamed Omar Elfarok, consultant vascular and endovascular surgeon at the General Organization of Teaching Hospitals and Institutes in Cairo, Egypt, and Bethanie Noll, Clinical Specialist for SIGVARIS GROUP USA. Tracy Green, Clinical Manager for SIGVARIS GROUP UK brought in her perspective as a panelist with many years of experience working as a nurse specialized in lymphedema care in the NHS. Two eminent speakers discussed the important, underestimated and underdiagnosed topic of breast cancer lymphedema. The first speaker, Dr Linda T. Miller, founder and clinical director of the Breast Cancer Physical Therapy Center in Philadelphia, USA, shared her expertise in the management of breast cancer lymphedema.  

Edemas consist of a lymphatic and a venous component. A comprehensive breast cancer lymphedema programme should also include softening fibrous tissue by applying medical compression. Appropriately applied medical compression using various modalities and textiles impacts fibrosis and filtration by changing tissue pressure and minimising excess filtration.  The best short-term and long-term outcomes will be achieved by addressing skin fibrosis and managing filtration. 

Dr Vincent Singh Paramanandam, qualified cancer case specialist and certified lymphedema therapist at the University of Sydney, Australia, highlighted the advantages of combining two methods used to prevent chronic lymphedema: the prospective surveillance method and the prophylactic approach. The aim of prospective surveillance is to detect arm swelling at a subclinical stage and to intervene early, usually with compression sleeves. In the prophylactic approach, compression is provided immediately following breast cancer surgery to avoid arm swelling in women at high risk. 

A systematic literature review and meta-analysis indicated that participation in prospective surveillance with early management reduces the risk of chronic breast cancer related arm lymphedema. A randomised controlled trial demonstrated that the prophylactic use of compression sleeves reduced the incidence of arm swelling, delayed the onset of arm swelling, and did not negatively affect quality of life, physical function and breast or arm symptoms. A combined approach of educating women about lymphedema symptoms and prophylactic use of compression sleeves may prove beneficial for patients in low resource settings.

He completed his Doctor of Physiology degree at The University of Sydney in 2021. His thesis title was Effectiveness of compression sleeve in preventing breast cancer-related lymphoedema. He has completed MSc in Cancer Care with Distinction from Oxford Brookes University, UK. He has an MSc degree in Psychology and a bachelor's degree in Physiotherapy.

Vincent Singh has 20 years of experience in cancer physiotherapy. He has conducted Lymphoedema certification programs, been an organizing secretary for Cancer Rehabilitation workshops multiple times, Scientific committee member for the CanRehab conference (2017) and taught in the Onco-physiotherapy program at Tata Memorial Hospital, Mumbai, India.

Management of breast cancer lymphedema: An evidence-based and practical approach

She served as the founder and clinical director of the Breast Cancer Physical Therapy Center in the Philadelphia area for over 20 years. She has over 300 hours of training in lymphedema management and has been an international student of lymphedema, including training in the UK, Italy, Spain, France, and Australia. In April, 2010, she helped to establish The Breast Cancer Rehabilitation Program at the Sultan Qaboos University Hospital (SQUH), the Sultanate of Oman's leading teaching hospital. She recently returned from the Middle East, where she was again invited to the SQUH to teach an advanced edema management course to an interdisciplinary team of health care providers.

Dr. Miller has also taken her edema management skills into the venue of sports medicine and orthopedics. Internationally recognized, Linda has published several articles and book chapters on post-operative breast cancer rehabilitation and breast cancer lymphedema. She has also trained hundreds of therapists and nurses in her comprehensive approach to edema management.

Having obtained a nursing qualification in 1986, Tracy embarked on a career in Trauma at an A&E department before transitioning to general surgery. In 1990, she pursued training as a District Nurse and developed a strong interest in Lymphoedema. By 1995, she established a Lymphoedema service in her local Trust and became one of the country's first Macmillan-funded Lymphoedema Nurse Specialist. Over 22 years, she led and expanded the service, receiving the BJN Lymphoedema Nursing award in 2011 for a community Lymphoedema key worker service initiative. In 2015, she joined SIGVARIS GROUP as a Clinical Advisor. Tracy is currently working as a Clinician Manager for the SIGVARIS GROUP Britain, and throughout her career, she has been dedicated to publishing articles and advancing care for those with Chronic Oedema and Lipoedema.

Mr. Mohamed Omar Elfarok is a consultant vascular and endovascular surgeon who specializes in advanced endovascular and open surgery for critical limb ischemia and diabetic atherosclerotic disease. He also has extensive experience in complex endovascular surgery for aortic aneurysms and lower extremity occlusive disease, as well as hybrid surgical techniques for PVD, varicose veins, and dialysis access.

Mr. Mohamed Omar Elfarok earned his undergraduate degree at Ain Shams University 1983. He completed his postgraduate vascular surgery training at west midland vascular rotation in UK and he obtained fellowship of the Royal college of Surgeons of England 1994 and Royal college of Edinburgh same year After completing his training, he joined the General Organization of Teaching hospitals and Institutes in Cairo Egypt GOTHI.

Mr. Mohamed Omar Elfarok's research interests include surgical education, simulation, course design, and milestone skill training. He has written numerous research publications and given many presentations on his work. He is also an Examiner of the Royal College of Surgeons of England .
